Medical Evaluation $250/hr minimum
If you have a non-urgent medical issue and would like this evaluated, please send an email to evergreen@epcmn.org. Please note, NO chronic care will be addressed or managed.

2nd Opinion Consultation $250/hour
If you have a medical chart you would like reviewed by our team of primary care physicians, please send an email to evergreen@epcmn.org. One of the physicians will reach out to review your request and obtain additional information.
