Dr Vang is an experienced US civil surgeon and will be able to perform the medical exam portion that is required to be conducted as part of the application for adjustment of citizenship status.
Pricing
Refugees or fiancee visa who need I-693 form: $80
This option is only for those individuals who already received the medical exam overseas and are applying for adjustment of status and only need to have immunization record updated.
Please note, that the cost does not include cost for immunizations or chest x-rays or specialists who may need to submit additional information to complete your form.
Anyone who has not had a medical exam and needs to complete an I-693 form:
-Children less than 2 year old: $320
-Children ages 2-17: $380
-Individuals ages 18+: $400
This is for individuals who have been instructed by USCIS or their legal counsel that they require a medical examination and labs to be completed for the I-693 form. The cost includes a medical examination and required labs for each applicant. Outside labs will not be accepted, even if they were recently done per USCIS guidelines.
A signed original application will be sealed in envelope and a copy will be provided for your records. Lab results take 3-4 days to return, so expect 1 week after your examination date before your application will be completed.
